All personnel carried on board CCG vessels for more than 12 hours must complete a statement of medical fitness prior to sailing. Answering TRUE to any question below requires you to have a medical professional conduct a confidential assessment of yourself prior to sailing and this assessment requires an additional 21 or more business days for any Health Canada review. Federal government employees with a Health Canada Field Scientist Occupational Standard certificate are exempt from the conditions below, but are bound by any conditions stated on their Field Scientist Occupational Standard Certificate. Note that government of Canada employees are required to have medicals competed every five years (age <39), every three years (age 39-46), and every two years (age 47+) although additional medicals may need to be required due to operational requirements.

Since my last health assessment, I have undergone treatment, or have consulted a health practitioner, for symptoms related to: heart; lungs; blood vessels; high blood pressure; dizziness; shortness of breath; muscle weakness; persistent aches or pains; or, blurred vision.

I have a history of seizures.

I have fainted or have lost consciousness during the past 12 months.

I am under the age of 40 and have not had a full physical examination within the last 36 months
OR
I am between 40-64 and have not had a full physical examination within the last 24 months
OR
I am 65 or over and have not had a full physical examination within the past 12 months.

A medical practitioner, within the last 48 months, recommended restrictions on my activities.

I have an existing physical or mental condition that is unable to be corrected by prosthetics, eyeglasses, or hearing aids, thereby adversely affecting my ability to walk, to climb, to see, or to hear.
